Why Your Night Guard Isn’t Fixing Your Jaw Pain

If you’ve been dealing with TMJ disorder for months or years, you’ve probably tried the standard approach: a mouth guard from your dentist, some jaw exercises, maybe pain medication when it gets really bad.

And perhaps it helped – for a while. The grinding stopped damaging your teeth. The exercises gave you some relief. But here you are, still dealing with jaw pain, clicking, tension that gets worse when you’re stressed.

Sound familiar?

You’re not alone. And more importantly, you’re not stuck with this forever.

The issue isn’t that conventional TMJ treatment doesn’t work. It’s that it addresses symptoms without asking a fundamental question: Why is your jaw so tight in the first place?

Why isn’t my mouth guard working?

Most TMJ treatment focuses exclusively on the jaw itself. This makes logical sense – after all, that’s where the pain is. You get a mouth guard to prevent grinding, exercises to strengthen the muscles, perhaps some massage for the tight spots.

These interventions are valuable. They protect your teeth, build strength, and provide relief. But they’re managing the problem, not resolving it.

Here’s what we see repeatedly at Hito: someone’s been wearing a mouth guard for a few years. Their jaws still click. It still occasionally aches during the day. It still locks up when they’re going through a stressful period at work. They’ve done everything their dentist recommended, and they’ve plateaued.

Your mouth guard isn’t failing you – it’s just incomplete. It protects your teeth from grinding, but it doesn’t address why your nervous system is creating that tension in the first place.

Then we assess them properly, and we find what’s been missing all along.

What’s Actually Driving My Your TMJ Pain

Your jaw doesn’t exist in isolation. It’s intimately connected to systems throughout your entire body.

Is my Nervous System causing my Jaw tension?

When your nervous system is stuck in a state of chronic stress – what we call sympathetic dominance or “fight or flight” – your muscles stay chronically tense. Your jaw muscles are no exception.

You might not even feel particularly stressed day-to-day. But if your nervous system hasn’t downregulated properly, perhaps from months or years of high pressure, past trauma, or chronic pain elsewhere in your body, that tension becomes your baseline. No amount of jaw exercises will release tension that’s being driven by nervous system dysregulation.

Moreover, the jaw muscles are part of the deep frontal line, the deepest muscle line in the body connected to our most ancestral and impulsive part of the brain (lizard brain), the one always on alert, very responsive to stress, negative thoughts and emotional traumas. If you ever experience any of these negative experiences, then your entire line is probably shrinking or tightening and it is prone to respond again with tension, over and over to any new potential trigger.

Can Fascia Restrictions cause TMJ?

Fascia is the connective tissue that runs throughout your entire body, linking everything together. Restrictions in the fascia of your neck, shoulders, or even your ribcage can create tension patterns that manifest in your jaw. Think of it like a sweater that’s been pulled tight in one corner – the tension shows up somewhere completely different from where the actual restriction is.

We often find that people with TMJ have fascial restrictions running from their temporal bones (the bones on the sides of your skull, underneath the ears) down through their neck and into their shoulders. The jaw is simply where this tension pattern becomes painful enough to get their attention. Often the jaws pain is associated with a tight diaphragm (shallow breath due to anxiety and stress), sense of restricted ribcage, short hip flexors (psoas/iliacus), tight calves (overall the deep tibialis posterior), sore feet medial arches (insertion of the tibialis posterior): they are all part of the same muscles chain: the deep frontal line.

My does my Jaw pain come with Neck pain and Headaches?

Old injuries, postural habits, breathing patterns, even how you sleep – all of these create tension patterns throughout your body. Your jaw might be compensating for restrictions elsewhere that you’re not even aware of. This is why TMJ often coexists with neck pain, headaches, or shoulder tension. They’re all part of the same interconnected pattern.

How is Hito’s Approach to TMJ different?

At Hito, we practice integrated physiotherapy. This means we respect and use evidence-based conventional techniques while also addressing the deeper patterns that keep your symptoms returning.

When someone comes to us with TMJ, we don’t tell them to throw away their mouth guard or stop doing their exercises. Those interventions have their place. Instead, we add what’s been missing.

We look beyond just your jaw. We assess your nervous system regulation, your breathing patterns, your posture, any restrictions in your neck and shoulders, and how your whole body moves and holds tension. This gives us a complete picture of what’s driving your jaw tension – not just where it shows up.

Alessio uses specialized techniques including myofascial induction therapy, acupuncture and craniosacral therapy to release fascial restrictions in your temporal bones, jaw, neck, and throughout your skull. This is gentle, precise work that addresses restrictions conventional physio often misses. Alessio integrates acupuncture into treatment sessions when appropriate, using fine needles at specific points to release fascial adhesions, calm the nervous system, and restore balance to the meridian pathways. Many clients find the combination of manual therapy and acupuncture creates deeper, more sustained releases than either technique alone. Craniosacral therapy works with the subtle rhythms of your cerebrospinal fluid and the membranes surrounding your brain and spinal cord. Myofascial Induction induces progressive and stable changes in the fascial tissues causing a renewal of the old restricted collagen fibres with new ones while transmuting the connective tissue matrix from crystalline almost solid (restriction) to gel-like or liquid (fluidity between layers and ability to glide). This is like performing a massive system reset which keeps improving for a long time, even after completing the sessions. For the TMJ, this allows us to release deep tension patterns that keep your jaw locked in chronic tightness.

We teach you techniques to help your nervous system downregulate from chronic stress states. This isn’t just about relaxation – it’s about giving your body permission to release the tension it’s been holding. When your nervous system calms, your jaw can finally let go.

We prescribe exercises based on your specific movement patterns and restrictions, not generic protocols. And we teach you how to practice them in a way that supports nervous system regulation, not just mechanical strength. How you breathe affects jaw tension more than most people realize, so we assess your breathing patterns and teach you techniques that reduce the load on your jaw and neck muscles.

This integrated approach – combining the best of evidence-based physiotherapy with specialized manual therapy and root cause treatment – is what creates lasting results.

What results can I expect from integrated TMJ treatment?

The people who come to Hito with TMJ have usually tried everything. They’re often skeptical that anything will actually work.

Then they experience what happens when you treat the whole pattern, not just the symptom. The jaw that’s been tight for years finally releases. The clicking that’s been constant for months resolves. The pain that’s been wearing them down fades away – and stays away.

Not because we’ve given them better symptom management tools (though those help), but because we’ve addressed what was actually creating the tension in the first place.

One client told us: “I’ve worn a mouth guard for three years. I thought I’d just have to live with TMJ. After working with Alessio, my jaw hasn’t clicked once in two months. I didn’t know that was even possible.”

This is what becomes possible when you treat causes, not just symptoms.

What can I do right now to help my TMJ?

While comprehensive treatment addresses the deeper patterns driving TMJ pain, there’s something you can do right now that often brings immediate relief: conscious jaw relaxation with breath.

Here’s how: Several times throughout your day – perhaps when you’re at a red light, making tea, or sitting at your desk – pause and notice your jaw. Is it clenched? Are your teeth touching? (They shouldn’t be when your mouth is closed and relaxed.)

Take a slow breath in through your nose. As you exhale, let your jaw drop slightly open, tongue resting gently at the bottom of your mouth. Let your face soften. Take three more breaths like this, each time allowing your jaw to release a bit more, gliding down and forward, possibly with the bottom teeth passing beyond the top teeth. If you feel comfortable and you don’t need to talk, drink or chew for a while, then you can hold it even longer for a few minutes of gentle but prolonged stretching, while consciously and slowly deep breathing. If the jaws muscles are like an elastic, if hold a stretching just for a few seconds while make the elastic return to the previous length, holding it for a prolonged time, beyond 90 seconds and up to 3, 5 minutes (even longer), then the “elastic”, at the end of the stretching will return to a longer and more relaxed position.

This simple practice does two things: it brings awareness to unconscious jaw tension, and it signals your nervous system to downregulate. Remember, you can’t heal what you can’t calm. This micro-practice of calming helps interrupt the chronic tension pattern, even if just for a moment.

It won’t resolve the root cause on its own, but it can help you start building a different relationship with your jaw – one where you notice tension earlier and have a tool to release it.

When should I seek integrated TMJ treatment?

If you’ve been managing TMJ symptoms for months or years, you might not even remember what it feels like to have a jaw that’s truly relaxed. You’ve probably accepted that this is just part of your life now.

But it doesn’t have to be.

Consider seeking integrated, root cause treatment if you’ve been using a mouth guard for six months or more with minimal improvement, if your jaw pain gets noticeably worse during stressful periods, if you have neck pain, headaches, or shoulder tension alongside TMJ, if standard physiotherapy helped initially but you’ve plateaued, or if you’re simply tired of managing symptoms and ready to actually resolve them.

Some of our clients come to us with TMJ as part of a larger picture – perhaps you’re also dealing with neck pain, tension headaches, digestive issues, hidden anger, frustration or persistent fatigue. When chronic symptoms show up across multiple systems like this, they’re often connected through the same underlying patterns of nervous system dysregulation and fascial restriction.

Our 12-week Hito Programme exists for exactly this situation. You work closely with our entire team – Alessio’s specialized manual therapy and craniosacral work, Lucy’s physiotherapy expertise, and Julie’s coaching, breathwork and nervous system regulation. It’s a comprehensive treatment that addresses your whole system, not just isolated symptoms, with personalized protocols specific to your pattern of restrictions.

Many clients tell us their TMJ was just one piece of a puzzle they’d been trying to solve for years. When we treat the underlying patterns driving all their symptoms, they experience shifts not just in their jaw, but in their energy, sleep, digestion, and overall sense of wellbeing. If that sounds like your situation, the Programme might be worth exploring.

Frequently Asked Questions about TMJ treatment

Why does my jaw pain get worse when i’m stressed?

Stress activates your sympathetic nervous system (fight-or-flight response), which causes muscles throughout your body to tense – including your jaw muscles. When stress becomes chronic, this tension becomes your baseline. Your jaw clenches as a protective response, even when there’s no real threat. This is why TMJ pain often flares during stressful periods at work or challenging life events. Addressing the nervous system dysregulation, not just the jaw symptoms, is key to lasting relief.

How long does it take to see results from integrated TMJ treatment?

Many clients notice some relief after their first or second session – often a sense of release or reduced tension they haven’t felt in years. However, sustainable change typically develops over 8-12 weeks as we address the deeper patterns driving your symptoms. The timeline depends on how long you’ve had TMJ, whether there are other chronic conditions involved, and your nervous system’s capacity to downregulate. Our 12-week Hito Programme is designed to create lasting transformation, not just temporary relief.

Can TMJ be cured, or will i always have to manage it?

When we address root causes – nervous system dysregulation, fascial restrictions, whole-body tension patterns – many clients experience complete resolution of their TMJ symptoms. This isn’t about “managing” the condition long-term; it’s about resolving the underlying patterns creating it. That said, TMJ can return if the contributing factors (chronic stress, poor posture, breathing dysfunction) aren’t addressed. Our approach teaches you the tools to maintain nervous system regulation and recognize early warning signs, so you can prevent symptoms from returning.

Do I need to stop wearing my mouth guard?

No! Your mouth guard serves an important protective function, especially if you grind your teeth at night. We don’t ask you to stop using tools that are working. Instead, we add what’s been missing – root cause treatment that addresses why your nervous system is creating the grinding and clenching in the first place. Many clients find they naturally stop needing their mouth guard as their nervous system calms and jaw tension releases, but this happens gradually as part of the healing process.

Will Acupuncture hurt for TMJ treatment?

Acupuncture uses very fine needles – much thinner than injection needles. Most people describe the sensation as a slight pressure or tingling rather than pain. When Alessio works on TMJ-related acupuncture points, you might feel a brief sensation as the needle releases a restriction, followed by a sense of relief or relaxation. Many clients find acupuncture deeply calming and actually look forward to it as part of their treatment. If you’re needle-sensitive, we can discuss this and adjust our approach.

Is TMJ connected to other health issues I’m experiencing?

Often, yes. We frequently see TMJ coexisting with neck pain, tension headaches, migraines, shoulder tension, digestive issues, and chronic fatigue. These aren’t separate problems – they’re often different expressions of the same underlying patterns: nervous system dysregulation and fascial restriction. This is why integrated treatment that addresses your whole system often creates improvements not just in your jaw, but in your overall wellbeing, energy, sleep, and digestion.

What’s the difference between standard physiotherapy and integrated physiotherapy for TMJ?

Standard physiotherapy typically focuses on the jaw joint itself – exercises to strengthen muscles, manual therapy to release local tension, and advice on managing symptoms. Integrated physiotherapy includes these evidence-based techniques AND addresses the deeper patterns: nervous system work to calm chronic stress responses, fascial release throughout your entire body (not just the jaw), acupuncture to balance energetic pathways, breathwork to support nervous system regulation, and personalized protocols based on YOUR specific pattern of restrictions. It’s comprehensive rather than symptom-focused.

How do i know if i need root cause TMJ treatment?

Consider integrated treatment if: you’ve been wearing a mouth guard for 6+ months with minimal improvement, your jaw pain noticeably worsens during stressful periods, you have neck pain, headaches, or shoulder tension alongside TMJ, standard physiotherapy helped initially but you’ve plateaued, clicking or locking persists despite treatment, or you’re tired of managing symptoms and ready to actually resolve them. If any of these resonate, a discovery call can help determine if our approach is right for you.

Does the Hito Programme include ongoing support between sessions?

Yes. Unlike traditional physiotherapy where you’re on your own between weekly appointments, the Hito Programme includes ongoing support when questions arise, community connection with others on similar healing journeys, access to resources you can use between sessions, and guidance on daily practices that support your nervous system. We recognize that sustainable healing happens in the moments between appointments, not just during them.

Can i do the Hito Programme if i live outside London?

Our 12-week Hito Programme is designed for in-person treatment at our London Bridge clinic, as the specialized manual therapy (myofascial induction, craniosacral therapy, acupuncture) requires hands-on work. However, we offer one-off consultations and can discuss adapted options depending on your situation. Book a discovery call to explore what’s possible for your specific circumstances.

Taking the First Step

The most important thing to understand is this: TMJ pain that keeps returning is your body telling you something deeper needs attention.

Your mouth guard, exercises, and pain management strategies aren’t failing you. They’re just incomplete without addressing what’s driving the tension.

At Hito, we practice comprehensive, integrated physiotherapy. We respect what’s working from your current treatment and add what’s missing – nervous system work, fascial release, root cause assessment, and specialized manual therapy.

We offer free 30-minute discovery calls where you can speak with our team, share what you’ve been experiencing, and learn whether integrated physiotherapy is right for your situation. There’s no pressure, no hard sell – just an honest conversation about what’s possible when you treat the whole pattern, not just the jaw.

Because you can’t heal what you can’t calm. And when we calm your nervous system and release those deeper restrictions, your jaw finally has permission to let go.
